**PLEASE READ ALL INFORMATION**

 

COVID 19 WARNING: IF ANYONE IN YOUR HOUSEHOLD IS COVID 19 POSITIVE OR HAS A CLOSE CONTACT EXPOSURE TO SOMEONE WITH COVID 19 WITHIN 14 DAYS OF YOUR APPOINTMENT, YOU CANNOT RECEIVE AN INFLUENZA VACCINE OR ATTEND THE CLINIC!


DEAR PARENTS AND PATIENTS: Our Pediatric Influenza clinics are by appointment only. We will hold a limited number of clinics this season, as our focus must be on patient care. However, we strongly encourage everyone to get vaccinated. If you cannot attend one of our drive-thru clinics, please make it a priority to get vaccinated at one of the many facilities that offer quadrivalent influenza vaccines.


PAYMENT: Prior to the clinics, please make sure we have accurate health insurance information on file. We will bill your health insurance for the vaccine and administration, statements will be sent for any unpaid claims, deductibles, or co-pays.


SAFETY: Please be patient during the clinic. We have tried to factor the time for every issue; however we will always prioritize safety and proper medical technique over speed. All of the RULES below are very important to our workflow and your safety. Please read carefully:


Drive-thru clinic rules (please come prepared):


  • Please do not attend the clinic if you do not have an appointment OR if the patient is sick.


  • Everyone in the vehicle, ages 2 years and older, MUST wear a mask over the nose and mouth to receive the vaccine.


  • The vaccine site must be easily accessible. Children 4 years and older should wear loose, short sleeves so the upper arm is exposedUnder 4 years of age should wear loose shorts or a onesie so the upper thigh is exposed.




  • Please arrive at your appointment time, to avoid vehicle back-ups. The patients should remain in their car seat, booster seat or seat belt. Due to the confined area in a vehicle, we will not attempt to vaccinate patients that are extremely uncooperative and require extra restraint.


  • Patients under the age of 18 yrs. must be accompanied by their parent or guardian. If the patient getting vaccinated is driving, we will direct you to a parking area to allow 10 mins recovery time.
