* PLEASE MAKE SURE WE HAVE ACCURATE HEALTH INSURANCE INFORMATION PRIOR TO THE CLINIC.

MANDATORY: You must have a completed '2022 Influenza Vaccine Screening Form' (for each child getting vaccinated) ready to turn in when you arrive at the clinic. Use the link provided below to print the form. You can also pick up blank forms at the office.

Drive-thru clinic rules (please come prepared):

1. Appointments are required. If your child is sick on the day of their appointment (fever over 100.4, sore throat, cough, or vomiting), please do not attend.

2. Everyone in the vehicle, ages 2 years and older, MUST wear a mask over the nose and mouth to receive the vaccine.

3. We need easy access to safely administer the vaccine, children 4 years and older should wear loose, short sleeves so the upper arm is exposed. Under 4 years of age should wear loose shorts or a onesie so the upper thigh is exposed.

4. Please read the 2022-2023 Inactivated Influenza Vaccine information sheet (VIS) prior to the clinic. The link is provided below. If you have any questions or concerns regarding the vaccine, please call the office or send a portal message at least 2 days prior to your appointment

5. Please arrive at your appointment time, to avoid vehicle back-ups. The patients should remain in their car seat, booster seat or seat belt. Due to the confined area in a vehicle, we will not attempt to vaccinate patients that are extremely uncooperative and require extra restraint.

6. Patients under the age of 18 yrs. must be accompanied by their parent or guardian. If the patient getting vaccinated is driving, we will direct you to a parking area to allow 10 mins recovery time.
